Qualitative Analysis

C2C helps in creating better discharge plans, improves quality and continuity of care, connections to primary health care, and reduces acute care use

"[C2C helps] discharge planning [by] connecting with the family physician [and] with CUPS […. C2C helps us in] coming together to [complete] complex discharge planning."

—Social Worker

C2C is working towards changing professional attitudes and stigma through better relationships between patients and healthcare providers

"[…] they are kind of breaking down some of the judgements on the medical side too by advocating for the clients so that people in hospital, paramedics and doctors understand some of the struggles that the clients are facing so it kind of breaks down the stigma"
